Sd card damaged. Reformat on my phone still damaged. What sh

currently im disappointed its been 3rd week since i bought this unit LENOVO A7000. i dont understand how suddenly it flashed on my notif.screen DAMAGED SD CARD!.. i have'nt put any harmful apps or even corrupted file or any..what i did today, i have backed up my files and tried to reformat many times in my PC using sd card adapter thru FAT32, exfat, and NTFS. NOTHING HAPPENED still my 8 GB is still damaged. but on my pc and other phones, sd card is readable.
can anyone help me.. ive done chkdsk many times, so i dont think my sd card is the problem coz my lenovo a7000 CANT READ ANY SD CARD anymore... its sad coz im afraid im gonna have to send it back to the company. any helpful ideas??
